PlaySafe is looking forward to providing physicals this Spring.  Please pay attention to the following information for dates, times, locations, and guidelines.  Please keep in mind that 100% of the money that comes from physicals for your school goes directly back toward paying for services and supplies for your athletic training needs. We will have physical forms (the back of the HSL form) to give to the athletes when they check in.

  • Athletes/Parents will register and pay the $20.00 fee, online with credit card, ahead of time (See Eventbrite ticket information below), print a ticket from registration site, and bring their ticket with them on the appropriate date. If they cannot pay online, they may pay onsite CASH ONLY.
  • Eventbrite ticket information: Go to Eventbrite.com, on the “looking for” line type in Playsafe, a dropdown box will open and you need to choose your county and make sure you choose the one for high school or middle school depending upon your child’s status. RISING 9TH GRADE STUDENTS NEED TO CHOOSE THE HIGH SCHOOL THEY WILL BE ATTENDING NEXT YEAR.
  • All Schools:
    • Athletes must bring with them the following:
      • Student ID or Driver’s License (if they have one or the other)
      • Ticket from online Eventbrite Physical Registration OR $20.00 CASH ONLY
      • Completed medical history form (front page of physical form) ONLY (we will provide the Physical Examination Form); either printed from their personal Arbiter account or filled out by hand (the doctors need this medical history in order to do a thorough physical and clearance).
      • HIPAA Form
      • If the athlete does not bring the completed medical history form with them, we will give them a hard copy form which will need to be completed and signed before they can have their physical done.

 

  • ***Overflow: Athletes who were not able to attend the first date or cannot attend the second date (due to a conflict with school events) This is a first come, first serve time slot. We will not continue to wait for athletes to show up so BE ON TIME.
  • We will make a copy of each athlete’s physical form before they leave the building
    • Our hope for this is that it will alleviate the problem of the physical being “lost” between the time they leave us and get home or back to the AD or AT.
  • As I am sure you are aware, if your school uses the Arbiter upload system, it will be key if you can go ahead and get your kids to go into their accounts and fill out all paperwork and get their parents set up to electronically sign as soon as possible after April 1.

 

PLEASE MAKE SURE THAT YOU DO NOT EVER CALL BLUE RIDGE OTHOPEDICS FOR INFORMATION ABOUT PHYSICALS OR TO GET A COPY OF A PHYSICAL. THEY DO NOT HAVE ANYTHING, DIRECTLY, TO DO WITH PHYSICALS. I WILL MAKE SURE THAT YOUR ATHLETIC TRAINER HAS A FLASH DRIVE WITH THOSE SAME PHYSICALS ON IT. THANK YOU.
